MEDICATION PROCEDURE

1. Medication Order form must be completed, signed, and dated by parent(s) and licensed health practioner (doctor, CNP, etc.) Form will be filed in child's health folder.

2. An Individual Care Plan will be developed by parent, Health/Nutrition Coordinator and/or physician. The Individual Care Plan will be placed in the child's health folder.

3. Medication should be:

a. bought in by parent or guardian; if not feasible, parent should contact the Health/Nutrition Coordinator who will contact bus driver and aide to notify of medication being transported.

b. in its original container with its name, health care provider's name and dosage.

4. Training will be provided to staff on administering medication by parent and Health/Nutrition Coordinator. Training shall include:

a. designation of staff member(s) to administer medication

b. form of medication (liquid or tablet)

c. quantity of dosage to be administered

d. possible side effects.

5. Administration of medication must be documented on reverse side on a daily basis.

6. Staff reports to Health/Nutrition Coordinator significant information about any side effects (ex child becomes lethargies, decrease in appetite, etc
